Read and initialize, in the box provided, on the 2nd page.
Each player requires a separate form, but you can make one (1) cheque to LAKESHORE FOOTBALL.
It is important that the e-mail address is clear and legible as this is the primary method of contact.
Sign and date form.
Include a cheque for the amount stipulated on the form for each age level addressed to LAKESHORE FOOTBALL.

Make a copy of form for your records.
Send envelope with form(s) and cheque to:

The appropriate age level coach will contact you once your form has been processed.
Please note:
Tryouts for AAA teams are held in spring (May - June) for the Mosquito and PeeWee levels, and while it is the intention to have as many players as possible, we can not guarantee a position on a team.
Lakeshore normally runs two (2) teams at each level, Atom, Mosquito and PeeWee dependant on registrations.
Atom and Tykes levels also have limits on the maximum number of players and registration is on first come, first served basis for these levels.
